All vendors must submit a Certificate of Insurance (PDF) document to Parkland Terrace HOA before work can commence.

Instructions

Vendors can submit the insurance certificate via email, or by completing the following form:

    * = Field is optional.


    Only numbers. Include area code. Exclude +1.



    Remaining Characters: 500

    This document must include a policy number, expiration date, and Parkland Terrace must be listed as an additionally insured entity (during the full duration of the project).
